Please note these awards are available ALL season.

The last award requests for the season will be accepted shortly after the District/State playoff/tournament.

To Request a USA Hockey Award (Hat Trick, Play Maker & Goalie-Zero Award) for your player(s):

1. Complete the Awards Request Form below.  This form is also available at http://www.minnesotahockey.org/page/show/894751-player-awards
2. Follow the instructions on the form and provide appropriate documentation - including a readable, unaltered copy of the score sheet. When completing the form please do not use abbreviations for the Association name, etc.   Please write out "Woodbury Area Hockey Club"
3. Once the form and supporting documentation is complete, please mail the form directly to the D8 Director using the address on the form please contact the WAHC Registrar at registrar@woodburyhockey.com to make arrangements to deliver the Request Form and docs to her.  The WAHC Registrar will then present the forms to the D8 Registrar.
4. Once the form is presented to the District 8 Director/Registrar, please allow 10-14 days for processing .
5. Patches will be mailed to the Coach/Manager as you indicate on the Awards form.
