The following are Avery templates that you may download and use for your team.  You will be required to provde FOUR (4) sets of rosters and coaching info for each game or scrimmage.  You are not required to use this format. 
Any 2" x 4" label works well.

Note:  Coaches' labels are no longer required.  However, the Head Coach must sign the scorebook before the game/scrimmage begins.
